This plan describes the program modifications and\/or adaptations for the student and the services that are to be provided. It serves as a tool for collaborative planning among the school, the parents, the student (where appropriate) and, as necessary, school district personnel, other ministries and\/or community agencies.<\/span><\/p>\n<p><span style=\"margin: 0px; color: #333333; font-family: 'Calibri',sans-serif; font-size: 14pt;\">It is a concise and usable document which summarizes the plan for the student\u2019s educational program. The IEP document does not describe every aspect of the student\u2019s program.
