Classroom Instructions/ Script on the day of the survey
TEACHER SCRIPT: PLEASE READ THESE INSTRUCTIONS ALOUD TO THE CLASS
Thousands of students, just like you, from over 80 high schools across British Columbia, Alberta, and Ontario have been chosen to take part in the COMPASS study. You will be asked to complete a health questionnaire. Questions in the survey are about different health topics including: physical activity, healthy eating, smoking, alcohol use, drug use, sedentary behaviour, mental health, and other behaviours related to health. It also includes some questions about how youth are dealing with the COVID-19 pandemic. This important project will help researchers, from the University of British Columbia to gather information about health topics among Canada’s youth. This information is also used to help plan new initiatives and evaluate current programs at our school. Your help today is very important!
Only students whose parents have given their permission, and who themselves agree to participate, will be doing the survey today. If you decide not to complete the survey, just don’t open the link. Students who are not doing the survey may work silently on ________________.
(Note: Classroom teachers should select something for these students to work on during this time.)
If you are doing the survey today, you will need a phone, laptop, tablet, or other device. As you complete the survey questions, please be aware that all of your answers are private. Your name does not appear on the survey. Your answers to the questions on the first page of the survey create a code that is unique from all codes created by other students participating in COMPASS. Because your name will never appear on the survey, your name will never be linked to the code you generate or to answers you give in the survey. This means that no one, not ever parents or teachers, will ever know what you answered. So, please be honest when you answer the questions.
If you do not understand a question or if you do not want to answer a question, please skip that question. I (teacher) cannot answer any questions you may have about the survey because the entire survey is private. During the time that you are working on the survey, I will stay at the front of the classroom so that I cannot see any of your answers. It should take about 30 minutes for you to complete the survey. You are free to withdraw from the survey at any time.
Once you check your email to open the survey link you may begin. Please work by yourself and quietly; as if this were a test. Once you are finished, submit your survey.
Thank you.
